When I was in Iraq we went and picked up a doctor and brought him back to our patrol base. When we got to the front gate everyone cleared their weapons and pulled the trigger in a clearing barrel. The Doc pulled the trigger and it went bang, he racked the slide again, pulled the trigger and it went bang. By this time two of my NCOs were already running up to him and grabbed him before he could do it again.
His specialty; brain surgery.

I saw this happen more then once. Sometimes brain-farts just happen... That is why there are clearing instructions at every clearing barrel in the army.
The one AD I have ever had was racking the slide, dropping the mag and dropping the hammer...
won't happen twice....
Slam fires are a bitch, cant really predict em either, something junks up your firing pin or gets it bent out of shape and you got to chamber a round and BOOM.
